This is so that Dr.Sherrell will be able to go over and discuss these records and test results with you during the consult and to aid in determining the correct diagnosis and plan of care for your condition.
It will allow Dr. Sherrell the opportunity to spend more time addressing your concerns and discussing with you the diagnosis and plan of care for your condition. If you have not had any of these tests completed recently by another provider,
do not worry as Dr. Sherrell will order any additional tests he may require for your specific condition
- Recent dental x-rays, especially panoramic x-rays
- Any recent medical imaging reports of the head and neck (CT and MRI scans)
- Any previous reports of biopsies done in the mouth
- Any recent blood work results completed by your physician
